Mission Health Care Network (Mission HCN) is designed and led by physicians. Mission HCN’s overarching goal is to improve the health of patients through a clinically integrated network that delivers high quality healthcare while achieving cost savings through the use of evidence-based guidelines and care coordination.
Unlike any initiative we’ve experienced, this network will be governed jointly by community physicians and Memorial Health Care System, with the distinction that physicians will lead the network and committee structures. You will have a greater voice in designing clinical improvement programs, and the network will provide you with the platform to participate in new payment models and shared savings programs – all designed to allow you to earn incentives for cost and quality improvements while keeping the patient at the center of care delivery.
Mission HCN has committed resources to provide legal, business and technical advice to assure success in working with a variety of payers. Through the relationship with Memorial, Mission HCN will access national contracts and pricing to leverage its expertise and tools to ensure success under the new payment models. This capability provides a cost effective way for Mission HCN and our physician partners to implement information technology, care coordination, and data analytics resources. Our network physicians will have the clinical information and support they need to provide efficient and effective patient care.
