Advance Directives

Everyone has the right to make his/her own health care decisions.

Advance Directives, also known as an Advance Care Plan or a Living Will, can help patients communicate their treatment choices if they are otherwise unable to make such decisions.

An Advance Care Plan is a document that allows patients to tell their doctors and loved ones how they want to be treated if they are terminally ill or permanently unconscious. Click on the links below to learn more.
